Understanding BPH and When to Intervene

BPH results from hormonal changes that cause prostatic tissue to proliferate - compressing the urethra and obstructing urinary flow. While not a cancerous condition, untreated significant BPH leads to urinary retention, bladder damage, recurrent urinary tract infections, and hydronephrosis (kidney back-pressure).

Symptom severity (IPSS - International Prostate Symptom Score):

  • Mild (IPSS 0-7): watchful waiting; lifestyle modifications

TURP and HoLEP - The Key Procedures

TURP (Transurethral Resection of the Prostate): The traditional gold standard for prostate obstruction surgery for over 60 years. A resectoscope is passed through the urethra; obstructing prostatic tissue is excised in small chips using an electrical loop.

Key points:

  • General or spinal anaesthesia; 75-90 minutes
  • 2-night hospital stay; urinary catheter in for 24-48 hours post-op
  • Effective for all prostate sizes (very large glands may require longer operating time or open surgery)
  • Possible side effects: retrograde ejaculation common (80%); erectile dysfunction risk <5%; urinary incontinence rare
  • Durable outcomes: symptom relief lasts 10-15 years; repeat procedures may be needed for very large glands

HoLEP (Holmium Laser Enucleation of the Prostate): The modern laser-based evolution - increasingly regarded as the procedure of choice for large prostates and patients requiring definitive single-visit treatment.

Mechanism: The holmium laser is used to enucleate (surgically remove in anatomical planes) the entire adenoma (the obstructing central zone tissue) - rather than resecting pieces. The enucleated tissue is morcellated (chopped) inside the bladder and removed.

Advantages over TURP:

  • Can treat any prostate size - even 200g+ glands (which might otherwise need open/robotic surgery)
  • Lower blood loss - laser provides haemostasis as it cuts
  • Catheter time typically 24 hours (shorter than TURP)
  • Lower retreatment rate - entire adenoma removed
  • Can be performed in patients on anticoagulants (reduced bleeding risk)

Side effects similar to TURP: retrograde ejaculation common; erectile dysfunction risk lower than TURP; transient stress incontinence 1-3 post-op months (usually resolves)

Post-operative course (TURP/HoLEP):

  • Day 1: Continuous bladder irrigation through catheter; clear fluid IV

Haematuria (blood in urine): Mild blood in urine is expected for 2-4 weeks post-operatively - caused by healing of the resection surface. Significant bleeding requiring hospitalisation occurs in <5% of cases. Patients are advised to maintain high fluid intake (2-3 litres/day) during healing.

Retrograde ejaculation: The most common long-term change after surgical BPH treatment - semen passes backward into the bladder rather than forward. This causes no harm and no symptoms, but results in dry orgasm. It is not reversible and should be clearly discussed with patients who plan further fertility.

Standard pre-operative investigations include: PSA (prostate specific antigen) blood test, prostate ultrasound (volume and post-void residual), uroflowmetry (urine flow rate measurement), IPSS symptom questionnaire, and basic bloods including renal function. If PSA is elevated, prostate MRI and/or biopsy may be needed to exclude cancer before proceeding. BB Global Health will advise on what investigations to obtain at home before your Turkey visit.

Permanent urinary incontinence is rare after TURP or HoLEP (<1%). Transient stress incontinence (leaking on coughing or exercise) is common in the first few weeks after HoLEP (less so after TURP) as the sphincter adjusts - almost always resolves within 1-3 months. Pelvic floor exercises help recovery.

TURP (transurethral resection) removes obstructing tissue in chips using an electrical loop - gold standard for 60+ years, suitable for all sizes. HoLEP uses a laser to enucleate (remove in whole) the obstructing adenoma - more complete removal, lower retreatment rate, safer for large prostates and anticoagulated patients. HoLEP requires more advanced surgical skill. Both are excellent procedures; the urologist recommends based on prostate size and patient factors.

Yes - most patients are comfortable to fly home at 5-7 days after TURP or HoLEP. The advice is to avoid long-haul flights (>4 hours) for 10-14 days due to DVT risk (dehydration and immobility). Patients travelling to Turkey from European cities (3-4 hour flights) are generally safe to fly at Day 7. Long-distance travel patients may need 10 days in Turkey.

No - BPH treatment does not increase or decrease prostate cancer risk. BPH and prostate cancer are separate conditions arising from different zones of the prostate. All patients are screened with PSA and examination before BPH surgery to ensure cancer is not the cause of the symptoms.
